Dr. Hillary Cass

Dr. Hilary Cass has published her final gender review report after a four-year wait, following her interim report in 2022.

The Cass review found that critical thinking and open debate are fundamental elements in scientific and medical research. However, the Guardian reports she found  professionals in the industry ‘are increasingly scared to openly discuss their views on the treatment of children questioning their gender identity’.

An overview of the key findings said, ‘There is no simple explanation for the increase in the numbers of predominantly young people and young adults who have a “trans” or gender diverse identity, but there is broad agreement that it is a result of a complex interplay between biological, psychological and social factors.’

Conflicting views and clinical approach

In her review, Dr. Cass stated: ‘There are conflicting views about the clinical approach, with expectations of care at times being far from usual clinical practice.’

This, according to the report, has made some clinicians fearful of working with gender-questioning young people, despite their presentation being similar to many children and young people presenting to other NHS services.

‘An appraisal of international guidelines for care and treatment of children and young people with gender incongruence found that that no single guideline could be applied in its entirety to the NHS in England’, it added.

Not enough evidence

The gender review further states that while a considerable amount of research has been published in this field, systematic evidence reviews demonstrated the poor quality of the published studies, meaning there is not a reliable evidence base upon which to make clinical decisions, or for children and their families to make informed choices.

‘The strengths and weaknesses of the evidence base on the care of children and young people are often misrepresented and overstated, both in scientific publications and social debate.

‘The controversy surrounding the use of medical treatments has taken focus away from what the individualised care and treatment is intended to achieve for individuals seeking support from NHS gender services.’

No rationale for puberty blockers

Christian Voice has always maintained that there is no justification for the use of puberty blockers, regardless of who it is being administered to.

The Bible says,
Luke 5:31-32 And Jesus answering said unto them, They that are whole need not a physician; but they that are sick. I came not to call the righteous, but sinners to repentance.

‘The rationale for early puberty suppression remains unclear, with weak evidence regarding the impact on gender dysphoria, mental or psycho-social health. The effect on cognitive and psycho-sexual development remains unknown,’ according to Dr. Cass’ findings.

‘The use of masculinising / feminising hormones in those under the age of 18 also presents many unknowns, despite their longstanding use in the adult transgender population. The lack of long-term follow-up data on those commencing treatment at an earlier age means we have inadequate information about the range of outcomes for this group.

‘Clinicians are unable to determine with any certainty which children and young people will go on to have an enduring trans identity.’

NHS gender services refused to participate

It has been disclosed that the NHS gender services astonishingly chose not to take part in the research process of the Cass Review.

In an email to John Stewart, the National Director of Specialised Commissioning at NHS England and NHS Improvement, Dr. Hilary wrote: ‘It is therefore hugely disappointing that the NHS gender services have decided not to participate with this research. I am frustrated on behalf of the young people and their families that the opportunity to reduce some of the uncertainties around care options has not been taken.

‘Ultimately, the NHS is an evidence-based service with a responsibility to maintain the safety
of those in its care. Understanding more about what support people who attended the Gender Identity Development Service (GIDS) received, and whether this helped them, will provide vital evidence to assist young people, their families, and the clinicians working with them to make informed decisions about the right pathway for them.’

She requested that NHS England consider how the objectives of this study could be achieved even after the completion of the Review.

Rishi Sunak, responding to the final review, said: ‘The findings have “shone a spotlight” on the need to “exercise extreme caution”,

The prime minister told LBC : ‘We care above all about the well-being of children and it’s clear that these things are not neutral acts, whether that’s social transitioning, any kind of medical intervention, we simply do not know the long-term effects of these things.

‘And that’s why anyone involved in considering these issues, of course, has to treat people with sensitivity and compassion, but also have to be extremely cautious when it comes to taking any action.’
